We’ve won an award! We’re very proud of our High Intensity User Service as they become proud winners of the “What Matters Award” at the South West Integrated Personalised Care Awards.
Through the High Intensity User Service, (HIU), caseworkers work one-to-one with frequent users of NHS services to help them access alternative support.
Our friendly and approachable HIU Co-ordinators listen to the needs of the client, supporting them to find solutions to their issues.
Our aim is help to improve the client’s quality of life, support them to reconnect with their families and community and help them to achieve their goals.
The service is spread across partner agencies, Volunteer Cornwall, Pentreath Ltd, and The Chaos Group. We are so proud of our 2 staff team members that work for the High Intensity User Service, supporting people in Cornwall.They are so dedicated and passionate about their work. Making a difference to people is what drives them. Well done to everyone involved!
