Current Opportunities
High Intensity User Caseworker
Hours of Work: 9-5 (37.5 p/w)
Base: Bodmin
Area of Work: Cornwall (flexible to the needs of clients)
Salary: £22,000 per annum
About the role:
The post holder will work with a rolling caseload of high intensity users, and through direct contact, seek to appropriately reduce the number of emergency ambulance calls and unscheduled care interactions. Improvements will be used to deliver measurable outcomes for the benefit of patients, staff and the public.
The main focus groups include some of the most vulnerable, homeless persons, people who self harm, the isolated and medical, social presentations who may not be accessing scheduled services and therefore rely heavily on unscheduled services.
Service personalisation is pivotal to this role, with concordance underpinning changes in patient behaviour rather than compliance through fear of isolation from supportive services.
We aim to find out what matters to people rather than what is the matter.
Alongside the individual, we create a bespoke plan and support the journey, reducing barriers and aiding navigation of pathways across all sectors and services. We strive to aid connection to a better life for the individual within their community.

Wellbeing Navigator
Hours of Work: 9-5 (37.5 pw)

Salary: £21,500.00 p/a
About the role
Cornwall Mind and its predecessors have run a range of wellbeing services supporting those in the local community that have struggled with their mental health for many years. They have usually been based around an activity such as art, walking, gardening and creative writing. Many of these groups have been long term offering much needed support to local people with sometimes complex mental health difficulties.
We are developing the service and its reach to cover more areas in Cornwall. The groups offered will range from time limited recovery and resilience groups, longer term volunteer led prevention groups and community cafes. These will be delivered using a blended approach of face to face and online. All groups use the Five Ways to Wellbeing as the core approach to support.
Aims
- Logistical: To coordinate referrals for attendance at Mind wellbeing groups across Cornwall and IOS.
- Administrative: To be responsible for the completion of all project paperwork.
Overall Responsibilities
- To work with Management Team and Administrator to assess referrals, identify individual interest and match clients to suitable Wellbeing Groups.
- Work with Mind Wellbeing and Comms staff to ensure all groups are well attended.
- Develop the initial Recovery Action Plan with participants prior to attendance at the Wellbeing Group.
- Collect all referral information and wellbeing scores and input in to Lamplight CMS.
